I am looking for a PHP form generator that will allow me generation of
multi page, branch forms. What I need is a form that will modify
questions on the following pages, depending on the answer given on a
previous page.
Example:

Page 1.
Question:
Is your pat a dog or a cat?
Answer: (radio buttons for a cat and dog)

Page 2.
Question:
What is your dog's name?
or
What is your cat's name?
(Depending on the answer from the page one.)

I am not a programmer and I can't write code to save my life, so I am
looking for a generator that would be as intuitive as possible, and
easy to operate. Can someone recommend something?

the program you want is probably along the lines of Lime Survey
