I am suddenly getting 354 errors from SquirrelMail when I attempt to send.
This does not happen with other UAs so this may well be an SM problem, but after perusing the error description on the web, I'm still unclear as to what the error means. Sounds like your SquirrelMail is pretty broken if it's treating that as an error condition. Only 4xx and 5xx codes are errors.
