You say you can access it externally, but then you say you cannot access it
from outside? I do not understand your question.

What I can say is if you want to access it from the outside world by it's
FQDN name (such as mail.yourdomainname.com/exchange), and Exchange is
installed internally, you will of course need to have a domain name first,
create a hostname, such as 'mail', and give it the external IP of your
router. Then of course in your router you will create a port-remap for port
80 to the Exchange server.

Internally you can access it by single name (NeBIOS name), such as
http://TheExchangeServerName/exchange. However you cannot use this name
externally and will need the FQDN.
